The fill in a comforter usually outlasts the stitching that holds it in. Comforter seams are the first thing to go, and a small split at a corner that would take four minutes to sew becomes a drum full of loose down and a ruined comforter after one wash cycle. Checking for it takes less time than the wash does.

Where comforter seams fail first
Failures are predictable, which makes inspection quick once you know the four places to look.
Corners go first. A corner is where two seams meet, where the fabric is under the most tension when the comforter is pulled, and where people grab it to move it.
Edge binding is second. The strip of fabric wrapped around the perimeter takes abrasion from the mattress edge, the bed frame, and the drum, and once that thread wears the whole edge opens progressively.
Baffle seams are third and they are hidden. Baffles are internal walls of fabric creating the chambers that hold the fill, and when one fails the fill migrates from chamber to chamber. You see it as a lumpy comforter with a cold empty patch.
Channel stitching on a sewn through comforter is fourth. Those lines pass right through the shell, and a broken thread lets fill escape sideways or out through the needle holes.
Zippers and closure tabs on duvet style shells fail on their own schedule and take the surrounding fabric with them when they do.
Why washing turns a small tear into a large one
A dry comforter lying on a bed is barely stressing its own stitching. A soaked one in a turning drum is a different object entirely.
Water is heavy. A wet comforter can weigh several times its dry weight, and all of that weight hangs off the seams as the drum rotates.
Agitation pulls the shell in directions it never moves on a bed, and a weak seam gets flexed hundreds of times in a single cycle.
Wet fill balls up into a dense mass instead of staying a distributed layer. That mass shifts around inside and shoves hard against whatever seam it lands against.
At spin speed the entire load presses outward against the drum wall, and a thread already broken in one place unzips along the line.
Then the loose fill goes everywhere. It clogs the drum, it coats the rest of the load, and a wet feather is remarkably difficult to get out of anything.
Inspecting a comforter before it goes in
This takes about two minutes and it is the entire preventive step.
Comforter seams give warning before they open, so spread it flat and run a hand around the whole edge binding, feeling for a soft spot where the thread has given but the fabric has not.
Check all four corners specifically, on both faces.
Hold it up to a window. Thin spots and pinholes in the shell show as light coming through, and a shell that has gone thin will tear during a cycle even where the stitching is sound.
Shake it gently and watch for down or fiber escaping. A comforter that leaks when shaken will leak far worse once it is wet.
Feel for uneven fill. Empty patches next to dense ones mean a baffle has already failed internally, and washing it will only make the distribution worse.
Look along the stitching lines against the light for a run of missing stitches, which is a seam in the middle of failing rather than one that has already gone.
Repair before washing, never after
This is the rule and it is worth stating plainly. A tear repaired before the wash is a small job. The same tear repaired after is a comforter with fill in the wrong chambers and a drum to clean out.
Fill that has migrated out of a chamber does not go back in evenly, and it will not redistribute itself later.
A wet tear is also harder to see and harder to sew, and a damp shell puckers under a needle.
If you find damage on a comforter that is already loaded, take it out and deal with it rather than running the cycle and hoping.
Push escaping fill back inside before you close the seam. Work it in from the outside with your fingers rather than stuffing more in through the hole.
Hand stitching that actually holds
Hand sewing is the right tool here. A home machine struggles with the bulk, and forcing a thick comforter through one usually gives you a puckered seam and a line of needle marks that becomes the next weak point.
Use a strong polyester thread rather than cotton, and a sharp needle sized to the shell fabric rather than the largest one in the tin.
For a split along an edge seam, a ladder stitch works well. Small stitches taken alternately on each folded edge, then the thread pulled so the edges draw together, and the stitching disappears inside the seam.
For a raw edge or a closure that needs to be secure rather than invisible, a whipstitch is faster and stronger. Small close stitches passing over the edge, and close spacing matters far more than neatness.
Start and finish at least an inch beyond the visible damage on each side. The thread next to a break is usually already weakened.
Knot securely at both ends and bury the knot inside the seam so it cannot catch.
Go over the repair a second time on a corner. Corners take the most force and a single line of stitching there tends to fail again.
Mending tape and other temporary fixes
Iron on mending tape is a reasonable emergency repair and a poor permanent one.
It holds with a heat activated adhesive, and dryer heat plus tumbling plus wash water is a fairly direct attack on that adhesive.
Down shells are usually tightly woven and often lightly coated, which is the surface adhesive bonds to worst. The tape lifts at the edges after a cycle or two.
It also creates a stiff patch, and fabric flexes at the boundary between stiff and soft, which is exactly where the next tear starts.
Fabric glue has the same problems and adds a hard line of its own.
Safety pins are worse than nothing in a wash, since they work loose and tear the shell as the load moves.
If tape is all you have before a wash, use it and treat it as buying one cycle, then sew it properly afterward.
Duvet covers and mesh bags protect comforter seams
A duvet cover is the single best thing you can do for comforter seams, and it works on the bed rather than in the machine.
The cover takes the body oils, the abrasion, and most of the wash cycles, which means the comforter itself needs washing a fraction as often.
Fewer wash cycles is directly fewer chances for a seam to fail.
For a comforter with a repaired or a suspect seam, washing it inside a cover or a large mesh bag limits how far a failure can spread and keeps escaped fill contained rather than loose in the machine.
A bag also stops a corner catching on anything inside the drum.
None of this substitutes for the repair. It limits the damage from a failure rather than preventing one.
When a comforter is finished, and how to avoid getting there
Some are past repair, and it is better to know that before spending an afternoon on one.
A shell gone thin and translucent across large areas will keep tearing somewhere new no matter how many seams you fix.
Multiple failed baffles mean the internal structure is gone, and the fill will never sit evenly again.
Fill broken down into dust and small fragments does not recover loft and cannot be redistributed into anything useful.
An odor that survives a proper wash and a full dry usually means moisture reached the fill and stayed there.
Preventing all of this is mostly handling. Lift a comforter with both hands under the body rather than dragging it by one corner, never wring it, support the weight when you pull it wet out of a washer, and shake it out to redistribute fill rather than tugging at the edges.
Wash it less often by using a cover, and dry it on low heat with enough time, since long hot cycles put more wear on comforter seams than the washing does.
The Raleigh part
The climate here works against comforter seams in a specific and slightly unfair way.
Humid summers mean anything thick dries slowly. A comforter that would take one dryer cycle in a dry climate often takes two or three here, and every one of those cycles is more tumbling, more heat on the thread, and more flexing of the same stitching. The drying, rather than the washing, is where most of the wear happens.
Heavy spring pollen is the second factor. It settles on everything, gets tracked into bedrooms, and pushes people to wash bedding more often through the spring than they otherwise would. More washes is more cycles on the same comforter seams, which is a strong argument for a duvet cover that can absorb those washes instead.
Moderate winters mean a comforter here goes in and out of storage rather than staying in constant use, and folded storage puts a sustained crease along the same seam lines for months at a time. Store it loosely in a breathable bag rather than compressed in plastic, and fold it differently than you did last year so the crease is not always in the same place.
Where a repaired comforter has room to move
A comforter that has already been mended needs a machine that is not forcing it into a tight space.
